Optik TV terms of service amendments

The following amendments to the Service Terms come into effect on October 25, 2009.

1. Section 2(e) of the Agreement is amended by replacing the words “at the prime rate of the Bank of Montreal plus 7%, converted to a monthly compound rate and rounded up to the nearest quarter percentage point, calculated from the due date” with the words “of 2% per month (26.82% per year), calculated from the billing date for those charges”.

2. Section 4(c) of the Agreement is amended by adding the following paragraph at the beginning of the section:

   Upon the expiry of the term of this Agreement, TELUS will continue to provide you the Services on a month to month basis on the terms and conditions then applicable, unless you notify TELUS at least 30 days in advance of your intention to terminate your Service or enter into a new service agreement.
