Pre-Authorized Debit (PAD) Agreement (“Authorization”)

Effective as of August 1, 2023

  1. I/we authorize TELUS, and the bank or financial institution designated (or any other bank or financial institution I/We may authorize at any time) to begin deductions as per my/our instructions for regular recurring payments, for payment of all charges for services and products provided with respect to the referenced TELUS Account Number(s) or Telephone Number(s). I/ we confirm that I/we have the authority under the terms of my/our account agreement to authorize the debit.

  2. I/we shall inform TELUS, in writing, of any change in information associated with the account provided in this Authorization at least fifteen (15) business days prior to the next due date of the pre-authorized debit.

  3. I/we acknowledge that I/we will continue to receive bills for services and products provided with respect to the Telephone Number(s) and/or TELUS Account Number(s).
    I/we acknowledge and agree that receipt of a TELUS bill serves as notice of the date of the debit and the amount to be debited from my/our Account and that the notice may, in some instances, be received less than ten (10) calendar days before the date of the debit or after the date of the debit. I/we waive the right to be pre-notified of a change if the amount debited from my/our account is different from the amount specified on the TELUS bill.

  4. This authority is to remain in effect until TELUS has received written notification from me/us of its change or termination. I/we may revoke my/our authorization by submitting a letter of cancellation to: TELUS, PO Box 2422 Station Main Edmonton, AB T5J 9Z9, fax a letter of cancellation to: 1-877-772-5446 or by visiting the TELUS website at

    . This notification must be received at least fifteen (15) business days before the next debit is scheduled. I/We may obtain a sample cancellation form, or more information on my/our right to cancel a PAD Agreement at my/our financial institution or by visiting

  5. I/we acknowledge and agree that TELUS may terminate this Authorization at any time verbally or by written notice delivered to you. I/we acknowledge and agree that TELUS may charge the Telephone Number(s) and/or TELUS Account Number(s) with a service charge for each dishonored payment as it occurs and that it may also result in termination or suspension of my/our participation in the Pre-Authorized Debit Plan.

  6. I/we acknowledge that revocation or termination of this Authorization does not terminate any contract for services or products that exists between me/us and TELUS. I/we acknowledge that this Authorization only applies to the method of payment and the amount of payment, and does not otherwise have any bearing on the contract for services or products including any related terms and conditions.

  7. I/we consent to the disclosure of any personal information that may be contained on this Authorization to the banking institution or financial institution, as far as any such disclosure of personal information is directly related to and necessary for the proper application of a pre-authorized debit.

  8. I/we have certain recourse rights if any debit does not comply with this agreement. For example, I/we have the right to receive reimbursement for any PAD that is not authorized or is not consistent with this PAD Agreement.   9. I/we understand and agree to all provisions in this authorization. I/we acknowledge that by reading these terms and by providing my/our information through the electronic PAD set-up process, I/we consent to these terms.
